The Ambulatory Care Pharmacist will work as a part of a multidisciplinary team in order to deliver comprehensive, safe, appropriate and cost-effective drug therapy for ambulatory patients with chronic medical conditions. Medication reconciliation, medication management and medication education will be paramount in ensuring optimal pharmacotherapy. Therapeutic areas of focus include CHF, COPD, AMI, stroke, hypertension, tobacco cessation, asthma, diabetes mellitus, anticoagulation and general medicine/pharmacotherapy.
Responsibilities
A day in the life of a Ambulatory Care Pharmacist at Hackensack Meridian Health includes:
1. Review medication profiles of patient seen in the ambulatory clinical setting (in person or virtually) and formulate patient specific medication-related treatment and monitoring plans
2. Perform medication reconciliation to determine appropriateness of medication orders and to assess patient adherence
3. Collaborate with physicians and APPs to optimize drug regimens by identifying, resolving, and preventing potential and actual medication-related problems
4. Deliver understandable and comprehensive disease specific and/or patient specific drug information to patients, caregivers, and health care providers involved in the care of the patient
5. Identify barriers to medication adherence and implement measures to facilitate access to medications, whenever necessary
6. Recommend appropriate lifestyle modifications in accordance with chronic disease state management
7. Develop trusting long-term relationships with patients
8. Provide education programs for physicians, nurses, pharmacists, patients, pharmacy students, and pharmacy residents
9. Precept pharmacy students and pharmacy residents
10. Represent the Pharmacy Department in committee meetings and hospital medication safety initiatives
11. Assisting in pharmacy and hospital initiatives (including readmissions reduction) to provide safe and efficacious evidence-based care
12. Perform research and utilize findings to optimize patient care and outcomes
13. Connect with the larger community by participating in wellness activities across the region
14. Other duties and/or projects as assigned.
