Job Description

The Registered Nurse answers calls to collect clinical information relative to a transfer request and facilitates the transfer by matching the patient's clinical need to the facility with service and bed capacity capability. The major responsibilities of the role are: provides status reviews for all patients from any portal of entry (planned or emergent) prior to bed assignment; performs initial review based on the status specified by the physician's order and will follow through with analyzing clinical information and making recommendations regarding appropriateness of care; assures the financial integrity of the organization by decreasing waste by assuring proper admission status is communicated to registration and care managers, that information regarding current admission is adequate to meet screening criteria or to communicate with physician and / or physician advisor when criteria for admission are not met. Assesses the patient to ensure the right care is delivered at the right time by identifying delays and removing barriers.

Responsibilities

Performs all responsibilities required by the department as defined in the scope of practice, to ensure that the unique nature of the client is addressed. Demonstrates the skills and judgment necessary to assure needs of the client and interdisciplinary team is addressed. Participates in unit based quality initiatives that help support process improvements. Oversees non-licensed staff to scope of practice and duties. Receives and directs phone calls and serves as the receptionist for the department. Promptly answers the phone, evaluates, and prioritizes incoming transfer requests calls for service while communicating effectively with all callers. Responds in a helpful way to all inquiries, as observed by staff. Gathers, presents, Enters and tracks appropriate clinical information in the computer. Thoroughly and accurately document all communication related to each transfer center call in a timely manner and within the timelines set forth within the policy/procedures. Demonstrate knowledge of EMTALA rules and regulations. Effectively triage all incoming requests for service according to the center's policies and guidelines. Manages multiple phone lines for discharges, admissions, room changes, doctor conferences, and transfer processes Operates a variety of office equipment effectively. Demonstrates the proper use of the fax, copy machines, computers, phones, etc. Collaborates with physicians, nurses, and other healthcare providers. Coordinate the updates with clinical staff made in electronic health record by efficiently managing the movement of patients through the care delivery process, including different facility locations. Maintains current knowledge/certification. Pursues professional growth and development.Assist in other duties as assigned to support the operational needs of the department and organization. May be required to remain on campus immediately before, during, and after severe weather and/or disasters.

Qualifications

Minimum Education

  • Technical Diploma/Certificate Nursing Required or
  • Associates Degree Nursing Required
  • Bachelor's Degree Nursing Preferred

Minimum Work Experience

  • 5 years Nursing experience in an acute care hospital Required

Licenses and Certifications

  • Registered Nurse Licensed State of Florida or eligible compact state Upon Hire Required
  • BLS for Healthcare Providers (BLS) American Heart Association Upon Hire Required

  • Demonstrated proficiency in acute care nursing, knowledge, and skills.
