SUMMARY

The Central City Concern Hepatitis C Elimination Program (CCCHEP) is an internationally-recognized program on the leading edge of the Hepatitis C elimination movement.

The Hepatitis C Program Manager is responsible for development, oversight, and optimization of CCCHEP across Central City Concern (CCC). The program manager is a member of the CCC Pharmacy Leadership Team, shaping CCC's Quality & Safety Plan, Population Health Strategy, and Clinical Standards. They will collaborate with medical and operations leadership to develop successful population health-based initiatives (e.g., Viral Hepatitis, HIV & PreP (Pre-Exposure Prophylaxis), and STIs).

MINIMUM QUALIFICATIONS (REQUIRED) For formal consideration for the Hepatitis C Program Manager position, please ensure your work/professional experience meets the following requirements:
  1. Bachelor's Degree in a relevant field.
  2. Three (3)-years of professional experience, minimum, managing in healthcare and/or related field.
  3. Expertise using Excel and other data management tools.
  4. Must possess a current driver's license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern's Fleet Safety policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training. Must maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.
Preferred Qualifications
  1. High competency in analyzing population health data, financial, and operational metrics.
  2. Experience with EPIC reporting tools.
  3. MBA/MHA or MPH.
  4. Experience with project management including certification with lean six sigma, acceleration process certificate.
ESSENTIAL DUTIES AND RESPONSIBILITIES: Leadership
  1. In collaboration with CCCHEP leadership and outreach team, develops, expands, and refines innovative programs aimed at eliminating hepatitis C in Oregon.
  2. Oversee daily operations for the team, including team huddles, co-outreach needs, coverage needs and staff support.
  3. Develops and maintains partnerships with outside community organizations who screen, refer, or facilitate treatment of hepatitis C through CCCHEP
  4. Accountable for the strategic planning, business model, clinical standards, and performance of HEP.
  5. Assists the Sr. Director of Pharmacy in planning, monitoring and/or managing budget for CCC's HEP
  6. Participates in various CCC projects or improvement teams to represent CCCHEP through active leadership.
  7. Complete annual performance reviews, implement corrective action plans when indicated.
  8. In collaboration with the Senior Director of Pharmacy and Hep C Medical Director, develop and maintain systems to ensure compliance with regulations, licensures, credentials, and contractual stipulations, including oversight and management of the patient records system.
  9. Identify and coordinate staff training needs and ensure the team remains committed to a harm reduction & trauma informed care
Program Oversight
  1. Manage HEP Team.
  2. Enhance Hep C program quality, assess, and improve accessibility of HCV treatment.
  3. Identify criteria to evaluate program outcomes, perform audits, and submit the evaluation to management.
  4. Facilitates Hep C planning, committee and research meetings, and staff meetings.
  5. Create organizational systems for Hep C programming, including the creation of policies and procedures as needed.
  6. Quality Improvement and Reporting: Coordinate with IT, Quality Management, and other stakeholders to implement and maintain Hep C treatment program data. Make recommendations from the analysis of this data. Track and report data related to patient program admission, progress, discharge, and other criteria as needed.
  7. Align other stakeholders within the HEP Team (e.g., providers, pharmacists, HCV outreach workers, HCV coordinators) and other stakeholders (e.g., Client Access, Primary Care, HR, IT Billing, and external partnerships.
  8. Analyze and review clinical, operation, financial and quality data to lead the program.
  9. Build new and maintain current relationships with community partners.
  10. Be accountable for program improvements via prioritizing projects in collaborations with appropriate stakeholders.
  11. Hiring and training HCV staff.
  12. Attends appropriate local, national, and global conferences.
  13. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  14. Perform other duties as assigned.
SKILLS AND ABILITIES:

  3. Solid understanding of harm reduction philosophy and experience working with marginalized populations, including people who use drugs

  14. Data tracking includes program outcomes for weekly, monthly, and quarterly reporting.
  15. Knowledge, education, training, skills, and experience in providing and supervising services in a highly diverse work environment to a highly diverse and mostly homeless population.
  16. Ability to work in an atmosphere where clients may be having episodes of acute psychological distress. knowledge of de-escalation methods or willingness to be trained in these methods.
