Location: Blackburn Primary Care Center - 12121 E. Burnside Street, Portland 97216

Schedule (Weekly): Variable (Monday - Friday)

Schedule (Hours): 0745-1715; 9-Hour Shifts

SUMMARY: This is an On-Call position. The RN nurse provides comprehensive community and clinic health services directed towards the prevention of disease, the promotion of health, and the client's self-care using professional judgment in comprehensive assessment, screening, nursing diagnosis, care planning, counseling, intervention, case management, health information /education, referral and evaluation.

***Note: On-Call employees must work at least once in every 3-month period to remain active on payroll.***

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Provide direct care and delegate care of patients as indicated by patient needs.
  2. Administer medications and treatments as directed by the standing orders and/or by physician. Utilize Pyxis and maintain electronic bed list.
  3. Observe and record pertinent information with regard to the care and condition of patients.
  4. Formulate individualized nursing care plans and participate in patient care conferences.
  5. Make referrals to hospitals and other agencies.
  6. Provide patient care and other work related directives for Subacute Technicians.
  7. Assist the staff physician. Respond in emergency situations.
  8. Give and receive report at shift changes.
  9. Consult with other health team members to lend guidance as necessary.
  10. Work closely with clinical staff ensuring integrated patient care.
  11. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  12. Perform other duties as assigned

SKILLS AND ABILITIES:

  1. Knowledge of nursing theory and practice.
  2. Knowledge of chemical dependency and treatment methods.
  3. Application of knowledge in nutrition, sanitation, and personal hygiene.
  4. Ability to follow verbal and written directions and to apply techniques for specific needs.
  5. Knowledge of community resources.
  6. Knowledge of de-escalation methods or ability to be trained in de-escalation methods.
  7. Sufficient manual dexterity and physical ability to perform assigned tasks.
  8. Ability to work in an environment where people may be hostile or abusive.
  9. Ability to manage time and meet deadlines.
  10. Ability to lift and move heavy objects.
  11. Ability to maintain accurate records and necessary paperwork.
  12. Ability to learn and apply training instruction.
  13. Knowledge of medical assessment with intoxicated persons.

MINIMUM QUALIFICATIONS (REQUIRED)

***Note: Candidates must ensure they meet the following requirements to be granted formal consideration for the On-Call Nurse position***

  1. Required: Successful completion of a course of accredited study that leads to the AD, or BS or AA degree in Nursing from OSBN or State Board-approved course.
  2. Required: Current RN license in the State of Oregon.
  3. Required: 6 months of nursing experience and/or recent educational preparation working with either psychiatric or chemical dependency populations. One-year experience with either population preferred.
  4. Required: Must be able to work to work a minimum of 3 days/month


PREFERRED QUALIFICATIONS

  1. BSN/QMHP.
